Enrollment Documentation needed Online Registration Paperwork, birth certificate, social security card, immunization record, photo ID of legal guardian, copy of lease or mortgage, and 2 proofs of residence. The mortgage/lease and 2 proofs of residence must be in the name of a legal guardian.
